Request the practice’s Notice of Privacy Practices
Contact the office at 404-913-7707 and ask for the current Notice of Privacy Practices and the person responsible for privacy questions. The notice describes the practice’s permitted uses and disclosures, its responsibilities, and how to exercise your rights. This page is a general summary, not a substitute for that practice-specific notice.
Your health-information rights
HIPAA generally provides rights to inspect or obtain copies of records, ask for corrections, request confidential communications, seek restrictions on certain disclosures, and receive an accounting of certain disclosures. These rights have conditions and exceptions. A legally authorized personal representative may be able to act for you.
Uses and disclosures
Health information may be used or disclosed for treatment, payment, healthcare operations, and other purposes permitted or required by law. Some uses require written authorization. Ask the practice for its notice to understand how these rules apply to its activities.
Questions or complaints
You can raise a concern with the practice or file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. HIPAA prohibits retaliation for filing a complaint.
