Tell the office which page or feature is difficult to use and the format or assistance you need. Do not include sensitive medical information in a website problem report.

Communication and language assistance

If you need an interpreter, an accessible format, or other communication assistance, contact the office before your visit when possible. Ask the team to coordinate the appropriate arrangements and explain the assistance available under applicable law.

Equal access to care

Patients have rights under applicable civil-rights and disability-access laws. If you have a concern about discriminatory treatment or access, contact the practice and ask who handles patient grievances. You can also contact the U.S. Department of Health and Human Services Office for Civil Rights.

Help with the visit

Call ahead if you need help finding Unit A1 or discussing access needs. The office is at 600 Houze Way in Roswell. For emergencies, call 911.
